Traceability: how to trace tests and defects, all the way back to requirements?

There are two levels of traceability in which you might might be interested:

1. Mapping each requirement to one or more tests to ensure that each requirement is covered.

2. Mapping each defect to one or more tests and perform a root cause analysis so that they can improve their processes.

AscentialTest Enterprise Edition is currently integrated with Jira (please see attached document for more information) and TFS for the second of those two requirements.
Each defect can be mapped back to one or more tests where users apply a reason code for the failure.
They can then run reports to determine the source of their defects.
The defects are typically attributed to the requirement, coding or test being the source of the defect.
Some people are surprised that the test can be the source of a defect.
It can be in the sense that the test might not be well designed and results in a false negative.

In future versions, AscentialTest will also integrate with TFS for Requirement Management, to support the first of the two requirements.
That will allow users to provide coverage analysis and trace defects all the way back to requirements. 

In addition, we also integrate easily with ALM/Quality Center for projects using that solution.
